An additional dose of pain reliever medication is often essential.

Synthetic hormones via contractions

The most commonly used induction procedures include infusions with the synthetically produced labor hormone oxytocin. This method, which usually quickly leads to violent contractions by continuously increasing the dosage, should only be carried out when the cervix is already soft or slightly open. In addition, a permanent CTG control of the child is indispensable until the contractions show the desired regularity.

It hurts and takes time – Activation of the plasticizer prostaglandin

The administration of the tissue hormone prostaglandin by tablet or vaginally by tampon, suppository, pessary or gel is also widespread; it also acts as a plasticizer for the cervix. The disadvantage of this form of introduction: Since the messenger substance does not usually take effect immediately, the treatment usually has to be repeated – until the birth process starts itself through the maturation of the cervix.

Egg pole detachment has a similar effect. The amniotic sac is detached from the cervix by manual action, so that natural prostaglandins are released and, in many cases, the birth starts within 48 hours. Although the procedure is very painful and lengthy, it offers the chance that no additional labor-promoting steps will be necessary.

Innovative and outdated methods

"Pretty new" explains midwife Friedrich, "is the initiation of childbirth with Dilapan-S. It is a thin stick that is inserted into the cervix like a tampon. It attracts liquid there and swells up accordingly." As a result, the cervix opens up through purely mechanical action and can therefore also be recommended for women with a shortening of the cervix or for women who have already had a caesarean section or another procedure on the uterus.

Risky and obsolete: amniotic sac rupture

The most risky, and now rarely used, introductory variant is the targeted manual opening of the amniotic sac, the so-called amniotic sac. Because with the "injury" The protective cover increases both the risk of infection for the child and the time pressure to complete an irreversible process as quickly as possible. If the desired start of labor does not occur with increasing inflammation levels, there is a risk of a caesarean section.

On the gentle route

Sometimes natural, less incisive measures can also help to get the labor pumping. For example, acupuncture, climbing stairs, going for a walk, relaxing baths or massages on the stomach or on the foot reflex zones can prove to be effective. Sex can be just as effective because semen contains prostaglandin. And the labor hormone oxytocin is released to a greater extent when the pregnant woman’s nipples are repeatedly stimulated.

Keeping calm helps

If there is no problematic indication, midwife Jana Friedrich advises affected mothers to stay as relaxed as possible when their babies are delayed and to trust the mechanisms of nature. In any case, the pregnant woman, as recommended by "obstetric guidelines" always the last word on the decision whether to initiate or not. It is therefore all the more important that the attending obstetricians are properly informed. "As long as everything is in order during the checks and (…) you still want to wait for a natural start of birth, you can do so and let your baby choose his own birthday."

When it comes to names for their baby, German parents love the classics – but not all will be called the same in kindergarten in the future. This is ensured by the great variety – including very unusual names. What was allowed: Kuddel or Urmel?

Marie and Elias are the most popular first names in 2016. Behind them are Sophie and Sophia and Alexander and Maximilian, as the Society for the German Language (GfdS) announced on Wednesday.

The most popular children’s names in 2016: Classics still popular

The parents remained largely true to their taste when it comes to first names for a baby: not a single newcomer made it onto the list of the ten names that are given most frequently last year. "The classics are the winners" says GfdS managing director Andrea-Eva Ewels in Wiesbaden. However, Elias secured first place in the ranking for the first time.

The GfdS evaluated more than a million individual names that were transmitted by around 650 registry offices. According to Ewels, this corresponds to around 97 percent of all given first names. There is no distinction between first and subsequent names.

Fips, Kuddel, Twain: unusual names are also allowed

In 2016, too, parents kept asking for unusual names for their babies. The registry offices gave their okay for Fips, Kuddel or Twain, among other things. "Parents don’t want their children harmed" says GfdS employee Frauke Rüdebusch. But many are not really aware that their sons and daughters also have to live with this first name as adults.

The company repeatedly rejects names in its reports, in 2016 for example Shaggy, Urmel, Ferrari or Bandito. In the vast majority of cases, the registry offices followed the verdict of the linguists, says Ewels.

But where does the desire for an exotic name come from? "The child is something very special for its parents – perhaps this should also be reflected in the name" says Rudebusch. However, GfdS managing director Ewels has concerns: "Children usually don’t want to attract attention."

Prof. Jürgen Udolph from "Center for Name Research" in Leipzig sees some unusual names "close to child abuse". Because it threatened bad teasing. "When a kid with a stupid name comes to school, it’s not always funny" warns Udolph. The arguments of the language experts meet with the parents sometimes more and sometimes less with open ears – sometimes they find an alternative together. For example, there was a desire to have the daughter "Prim" to be called, as Rüdebusch relates. The objection that this name could possibly match the negative term "primitive" associated, the parents would have accepted. Rudebusch struck "Primrose" before, the English term for "Cowslip".

An Islamic name is becoming increasingly popular in Germany

The Islamic-influenced Mohammed has recently made a big leap forward on the overall list of all names from the German registry offices – from position 41 in 2015 to position 26 last year.

In 2016, the ranking of Turkish-Arabic first names in Germany was led by Elif and Mohammed. Layla and Nour / Nur for the girls and Ali and Yusuf for the boys follow on the other places. This means that the tastes of parents in Germany partly differ from those whose children are registered in Turkey. To this end, the GfdS evaluated lists of names from the Turkish Interior Ministry. Accordingly, in Turkey, the girls Zeynep, Elif and Hiranur land in the top places, with the boys Yusuf, Eymen and Ömer.

Children with common first names do not automatically have numerous namesakes in kindergarten. Even Marie only had a share of 2.77 percent of all female first names awarded in 2016, Elias made 1.42 percent of male first names.

When choosing a name, Germans obviously care about the sound

Prof. Udolph has discovered a commonality in many popular names: "German parents choose it based on its sound, many vowels are important." According to the expert, it is questionable whether everyone in the family is permanently happy tyland brides with a name. Studies from England, for example, have shown that more than half of the parents’ couples were dissatisfied with the name they chose for their baby after just one year.

Here are the top ten for girls and boys and their positions from the previous year in brackets:

In medical terminology, placenta means accreta "grown plaster cake". It is a disorder of placental adhesion that can lead to dangerous complications after delivery. An expert explains what causes this abnormality, what the risks are, and what treatment options are available.

During pregnancy, the placenta ensures that the unborn child receives nutrients through intensive blood exchange, but is also protected from harmful substances. After giving birth, the placenta has done its job – in most women it therefore separates itself and is expelled in the postpartum phase.

Growing together with the surrounding tissue

However, this natural process does not always work, so that in some mothers parts of the tissue with a high blood supply remain in the body. "If the placenta in some places grows through the mucous membrane to the muscles of the uterus or on a scar, spontaneous detachment after the birth is no longer easy" explains Christian Albring, President of the Professional Association of Gynecologists.

"Heavy bleeding can then occur after the birth, for example because the muscle layer of the uterus is torn, or remnants of placental tissue remain on or in the wall of the uterus."

Dangerous blood loss threatens

In most cases, this disorder requires scraping the uterus after giving birth. On the one hand, this can stop the bleeding, and it also counteracts possible regression disorders, inflammation and growths, emphasizes Albring.

Women are almost always unprepared when complications arise for the first time because of a placenta accreta, as is the case with a young mother who reports in a forum about her experiences on the day her daughter was born.

"Although everything had been straightforward before, my placenta just wouldn’t come off after the delivery – it was somehow stuck. Then suddenly I was bleeding heavily and finally had to be taken to the operating room quickly and was scraped off under general anesthesia. I had lost a liter of blood. Fortunately, I was spared a blood transfusion."

Hysterectomy as a life-saving measure

Not everyone is as lucky as this new mom. The British media reported in the summer of a 38-year-old from Oxford who almost died giving birth to her fourth child. She lost more than five liters of blood due to placenta accreta, the newspaper wrote "Mirror".

If such an extreme situation occurs, the treating doctors have to react quickly. "The bleeding is profuse, and life-threatening for mother" says the experienced gynecologist Albring, "In many cases, the uterus has to be completely surgically removed." This measure also saved the mother of four from Great Britain.
